Social Drivers of Health: The Non-Medical Factors that Influence Health Outcomes

Social drivers of health (SDOH) are external factors that significantly impact a person's health outcomes and well-being. These factors include economic stability, education access, healthcare access, neighborhood and built environment, and social and community context. Our role as health care professionals is to recognize the drivers of health that impact the delivery of care and integrate practices to address SDOH.

Target Audience

Nurses, MDs, PAs, Social Workers, and other health professionals.

Learning Objectives

Upon completion of this learning activity, participants will be able to:

1. Define and describe the five domains of social drivers of health.

2. Explain how social drivers of health contribute to health inequities and population health outcomes.

3. Identify assessment tools and nursing interventions addressing social drivers of health.

4. Apply interprofessional strategies to improve health equity in their practice setting.

5. Reflect on one actionable change you can make in their role to address social drivers of health.
